Application Process

To begin the application process (new and renewal), visit your Medical Provider; they must be the one to start your application.

As a reminder, the Department of Health has no fee to enroll and participate in the Medical Cannabis or to register and use the Online Patient Portal.

The instructions below provide detailed information regarding the application process and use of the Online Patient Portal.

NOTE:   This system is compliant with the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) as amended by Health Information Technology for Economic and Clinical Health Act of 2009 (the “HITECH Act”), including the Standards of the Privacy of Individually Identifiable Health Information and the Security Standards at 45 CFR Parts 160 and 164.

09/01/23:

06/16/23:

Effective Friday, June 16, 2023, all Electronic Cards have a new look.  In addition, all applications processed from this day forward will have a 2 year expiration.
